As the years go by, you may feel as though the end of your nose has begun to wilt or sag. You’re not alone—many men and women seek out a doctor’s opinion on correcting a drooping nose tip. But according to a recent report, your nasal tip might not be the thing that’s actually changed over time.

Plastic surgeons with expertise in facial reconstruction discovered that the issue may in fact be caused by changes in the areas around the nose, rather than the nose itself. Suggesting that the appearance of a “drooping tip” may be an optical illusion, the study observed what areas of the face do indeed change the appearance of our nasal tip as we age.

If not the nose, who’s the culprit?

The participating doctors examined photos of twenty-five female patients who complained of changes in their noses over time. The study photos captured the participating women smiling as well as with neutral expressions. Details of each patient’s face were then measured, allowing researchers to precisely track changes over time.

The doctors discovered that the tips of the patients’ noses moved less than 1 mm on average and that the “plunging tip” was most likely an illusion caused by changes in other parts of the face. What did shift were areas surrounding the nose, including the nasolabial creases surrounding the mouth and the lower part of the septum.

Why does the tip of the nose seem to change in appearance with age?

Depletion of collagen, changes in our nasal cartilage, and loss of skin elasticity that occur with aging may be responsible for the appearance of a plunging tip. As collagen is depleted, the bones in our faces come closer to the surface of the skin. This, in combination with loss of bone mass in the jaw and upper teeth area, can cause the angle between the mouth and nose to change, resulting in the tip of the nose appearing to sag.

While changes in nasal tip appearance may not be the fault of your nose, there are a range of options for addressing the concern. There are both surgical and nonsurgical procedures for treating signs of aging in the area around the nose, which can in turn help you perk up the look of a drooping nose:

  • Rhinoplasty. While you nose might not actually have changed much over time, a Tip Rhinoplasty, which refines and raises the angle of the nose tip, is still a very effective way to correct the appearance of a drooping nasal tip.
  • Facelift. For those looking to address more than just the nose, a facelift can reduce many visible signs of facial aging by removing excess fat, tightening underlying muscles, and redraping the skin of your face and neck. The procedure thereby improves the appearance of nasolabial creases that affect nasal tip appearance.
  • Dermal fillers. Injectable fillers such as Juvéderm®, Restylane®, and Sculptra® are a great nonsurgical option for addressing problem areas around the nose, which may be the reason you’re seeing a nose droop.

        TD Five Boro Bike Tour—May 4
        Imagine riding a bike through all 5 boroughs and across 5 bridges—without traffic! Bike New York and the New York City Department of Transportation produce this annual event, when the roads are cleared of traffic and more than 30,000 riders experience the city on bikes. A stellar lineup of artists provides entertainment all along the tour’s route, culminating in a Finish Festival on Staten Island.

        2014 James Beard Awards Gala—May 5
        If you’re a serious foodie, this is not to be missed. The black-tie event at the David H. Koch Theater in the Lincoln Center honors the winners of the year’s James Beard Awards for culinary excellence. This year’s theme is “Sounds of the City.” The gala dinner chefs are each from an American city with a shared love of music and food.

        Charles James: Beyond Fashion—May 8 through August 10
        The Metropolitan Museum of Art’s Costume Institute exhibits are always spectacles. This year’s exhibit focuses on the work of Charles James, one of the most renowned designers to work in New York City; his last studio was in the Chelsea Hotel. This collection of 75 ball gowns is bound to be exquisite. The designs will be presented in two different locations in the Museum: the new Lizzie and Jonathan Tisch Gallery in the Anna Wintour Costume Center and in special galleries on the Museum’s first floor.

        Mother’s Day at the Central Park Zoo—May 11
        Come to the zoo to celebrate moms, both animals and humans. The family party has something for everyone and includes visits with animal moms and their babies; an animal mom’s scavenger hunt; and the opportunity to make tasty treats for the animals. Children will also make a special Mother’s Day craft. It has all the makings of a memorable day at the Central Park Zoo, which is not far from our office on Central Park West.

        NYC Craft Beer Festival-Summer International—June 20 & 21
        Better get your tickets now for this beer lover’s event! Held at the Lexington Armory, the summer show will focus on the best craft beers from home and abroad, with both American and international brews available for tasting. This showcase of brewing expertise will offer approximately 150 different beers, with food and retail vendors also on site.

        We have two more spring-related notes we’d like to share. Dr. Zevon is a passionate birder, and the spring migration is currently in full swing, with warblers and other neotropical species filling the treetops of Central Park. And we’re delighted to announce that the Tavern on the Green; has just re-opened; this historic Manhattan restaurant is known the world over, and it’s right around the corner from our office!         An eyelid lift, or blepharoplasty, is one of the most rewarding cosmetic surgery procedures, because the eyes communicate so much about us. When the eyes are affected by excess skin, unsightly bags, or dark circles, the entire face can suffer: you can look tired or worried when you are not. An eyelid lift can address the upper lids, the lower lids, or both, rejuvenating the entire face and making you look years younger.

        As dramatic as the changes from an eyelid lift might be, the surgery can’t resolve every issue with your eyes. Let’s examine what an eyelid can—and can’t—do, including removing dark circles under the eyes.

        An eyelid lift can:

        • Remove loose, sagging skin. Excess skin hanging over the eyes can make you look older and more tired than you actually are. An eyelid lift improves sagging eyelids by removing all the loose skin, opening up your eyes. Your peripheral vision can sometimes be improved from a blepharoplasty, too.
        • Remove bags under the eyes. As we age, fat can accumulate and create unappealing bags under the eyes. Puffy bags can be eliminated in an eyelid lift, smoothing and refreshing the upper face; fat can also be redistributed to address under-eye bags.
        • Remove excess fat. If your upper lids are also puffy, fat can be removed during your surgery. Excess muscle may also be removed.
        • Improve droopiness. The lower eyelids can sometimes droop below the iris, showing white under the colored portion of the eye. A blepharoplasty can correct this issue.
        • Restore definition to the eyelids. When sagging skin, loose muscles, and excess fat have impacted the shape of the upper eyelids, an eyelid lift can restore the youthful contour you once had.

         

        An eyelid lift can’t:

        • Remove dark circles. Because true dark circles under the eyes aren’t generally caused by the aging process, an eyelid lift can’t make a significant improvement in their appearance. One exception: sometimes what is perceived as a dark circle is actually a shadow cast by a puffy eyelid or a hollow formed under the eye from fat migration; this can be addressed through a blepharoplasty.
        • Remove crow’s feet. Crow’s feet—the wrinkles at the corners of the eyes—are best treated by non-surgical methods. BOTOX® is now FDA approved for relaxing and smoothing crow’s feet, and skin resurfacing technologies are also successful for addressing these areas outside the scope of an eyelid lift.
        • Address sagging eyebrows. While an eyelid lift surgery can brighten the eyes, a sagging brow must be approached with a different surgical procedure, a brow (or forehead) lift. This surgery elevates the brow and smooths wrinkles and creases in the forehead. Because a drooping brow can also adversely affect the appearance of the eyes, an experienced plastic surgeon should be consulted to help guide you toward the appropriate procedure for your aesthetic concerns.

         

        Although the natural aging process will continue, an eyelid lift can restore a youthful twinkle to the eyes and refresh your entire appearance. Come visit us to discuss what an eyelid lift can do for you.

        Working right on the edge of Central Park, we know a thing or two about how to get around New York. With St. Patrick’s Day fast approaching, thousands of people will flock to 5th Avenue to take part in the festivities. There is no doubt about it, New York hosts some pretty amazing parades and the St. Patty’s Day Parade doesn’t disappoint. Here is our advice as longtime New Yorkers:

        The Early Bird Gets to See the Show
        The St. Patrick’s Day Parade starts at 11:00 am, so get to 5th Avenue no later than 10:00 am at the latest. There is nothing worse than arriving on time to these events because there will be no place to stand and no one is going to let you move up.

        Keep On the Sunny Side
        If you haven’t visited New York in the later winter months, understand this: it’s cold. If you do manage to get to 5th Ave at a decent time, you can keep warm and catch some sun by standing on the west side (if it’s sunny of course).

        Coveted Standing Room
        The best place to see the St. Patrick’s Day parade is by standing anywhere along 5th Avenue north of St. Patrick’s Cathedral. Also on this side of town is a great little sandwich shop, tucked right behind the cathedral, called Toasties. You can order in advance and have provisions for the parade.

The breast is composed of glandular, fatty, and fibrous tissues positioned over the pectoral muscles. A layer of fatty tissue surrounds the breast glands, giving the breast its soft consistency. Breast size and shape can change due to circumstances in a woman’s life including pregnancy, aging, and fluctuations in weight.

        Because liposuction is effective in removing fat, it can be used as a reduction technique if the breasts contain a large degree of fatty tissue. Because the breasts of a younger woman contain more tissue and less fat, a liposuction-based procedure tends to be more successful on older women, but the reduction is generally modest, accounting for about one cup size. For some, this may be enough. But for many, breast reduction with liposuction will not it result in a sufficient change and the outcome is likely to be unsatisfactory.

        The Advantages of Traditional Breast Reduction
        Although breast reduction surgery does leave visible scars, it can do more than liposuction, namely reshape the breast and remove excess skin. Breast reduction surgery offers long-term results that improve the appearance of the breast and alleviate the back, neck, and shoulder pain from large breasts. By addressing the shape of the breast, breast reduction also helps counteract the effects of aging, lifting sagging breasts to a more youthful position on the chest. The results are permanent as long as a woman continues to maintain her weight and an effective exercise routine.

        Liposuction to Complement Breast Surgery
        While liposuction alone is not usually an ideal breast reduction technique, it is an ideal way to address fat deposits near the breasts. Even fairly slim women may have unsightly fat deposits near the bra line and next to their underarms, which will be even more noticeable once their breast size is reduced. Thus liposuction can be an important tool for your plastic surgeon to use along with breast surgery to create pleasing upper body contours.

        In sum, reduction surgery is still the gold standard for relieving women from the burden of overly large breasts. A consultation with a board certified plastic surgeon skilled in breast procedures can help you determine how to best achieve your goals for breast reduction.
